Dear Parents & Guardians,

Healthy Smiles is an innovative leader in children’s dental care that visit schools throughout New York State.
Our goal is to make oral health care accessible to every child. It is one of the few outreach programs fully
approved by the The New York State Department of Health.

Healthy Smiles, in collaboration with the NYC Dept. of Health and the NYC Dept. of Education, provides
preventative dental care by our highly trained dental team. The services provided as needed can include the
following: exam/screening, gentle cleaning, fluoride treatment, sealants, x-rays, and silver diamine fluoride
(this can help prevent existing cavities from getting larger). We also provide an interactive educational
workshop teaching children how to maintain proper oral hygiene. All treatments are noninvasive and done in
the school under the direct supervision of the school staff. A follow up report will be sent home explainingwhether the child has any cavities and if they need to visit their primary dentist for further care.

When you sign the consent form, your child can go to both the school based dental health center and your
primary care dentist. You will never be required to drop your primary care dentist. According to the American
Dental Association, exams, cleanings and fluoride treatments should be done once every six months. Once
the consent form is signed, it is active for the duration of your child’s enrollment in the school. Please inform
us in writing (at least 2 days before an upcoming visit) if you do not want your child to receive any further
services from our program. You have the option to attend your child’s dental visit to the SBHC-D in person or
be on the phone while your child is being seen. You can contact our doctors 24/7 whenever your child has a
dental emergency even when schools are closed or not in session by calling (718)-257-7777.

All dental services are completely covered by Medicaid or private insurance. There are never any out
of pocket costs. We can also assist uninsured families in obtaining insurance by connecting them to a
health insurance enrollment specialist.
